Access to Cadillac Ball Forms and Reports  (which includes adding prizes, viewing tickets sold, and table seating)

Instructions for online forms:

1. When you click on the Access to Cadillac Ball Forms you will need to enter the user name and password. Forgot the password? Email Tom Combs. If you experience difficulties with entering data contact Joe Bono. If you have questions or problems with "Adding a Prize" contact Kurt Westerlund.

2. Seating_Reservation Form For Rotarian's only. This is the same as last year.  Just print the form,  fill it out and give it to Laura Ryan.

3.Ticket Instructions

                                        i.      Get a ticket voucher from Alan Harry (remember they have no value and a ticket is not reserved until money is paid!) If the guest wants a vegetarian meal please write "vegetarian meal" on the voucher. If the guest is associated with a company table write the company on the voucher.

                                        ii.      Fill out ticket receipt completely (all fields)

                                       iii.      Yellow goes to guest, white to Alan Harry

                                        iv.      Payment goes along with receipt to Alan Harry

                                        v.      Payment will be deposited at citizens bank, and a ticket number will be assigned

                                         vi.      Notes: Each ticket should have its own contact (ie. one receipt per ticket).  First come first served. Tickets are not guaranteed until payment and ball receipt received by Alan and confirmed that seats still exist.

                                         vii.      Once ticket is assigned, the tickets will be mailed to the guest at the address specified on the receipt. Tickets will be mailed in batches, please be patient.

                    viii.  This is important for this year - There is really now way of knowing exactly how many tickets remain available towards the end of the tickets sales. Because the tickets are generated in real time. If you or your friends do not wish to be disappointed it is imperative that you PAY for the tickets earlier rather than later. You can look in the "Forms and Reports" site to get an estimate of how many tickets have been sold but as they say "the late bird get's nothing".

             ix.  Whole Table purchases - If someone wishes to purchase an entire table of 10 use can use the special Full Table Form. One check can be written for the table. Attach the Full Table Form to one  ticket voucher. Give the yellow copy to the customer. We need the names and addresses of the individuals at the table so that we can mail them their tickets. 

             4. Prize Instructions

Step 1: Print out the redemption certificate

 Step 2: Fill out the redemption certificate with you and your donor

Step 3: Then, fill out the prize form online at the password protected site "Cadillac Ball Forms and Reports". THIS IS A NECESSARY STEP.

 Step 4: If a signed redemption certificate form is NOT required, then you are done. If a signed redemption certificate is required, fax it to 550-0315
